Surgical Offloading of the Metatarsals (Ball of the Foot)
Most ulcers in the metatarsal region will heal through conservative care, centered on offloading and debridement.

Long term, most of these are managed long-term with custom orthotics, custom shoes, or off-the shelf footwear made for neuropathic ulcerations. Sometimes a CROW boot is used.

But sometimes surgery is necessary. One common procedure is to lengthen a tight Achilles tendon to diminish excessive pressure in this region.
Surgery can also involve surgery on the bones themselves.
